Electronic Visit Verification for New Jersey Home Care Agencies

New Jersey requires Electronic Visit Verification (EVV) for every Medicaid-funded personal care and home health care visit. Each visit must record six details electronically, and the data goes to the state’s aggregator, HHAeXchange. Here’s what agencies need to know, and how HCSG keeps your EVV records clean.

The rules

What does New Jersey require for EVV?

New Jersey requires EVV for every Medicaid-funded personal care visit (since January 1, 2021) and home health care visit (since January 1, 2023). Visits are recorded by app or landline call, the data goes to HHAeXchange, live-in caregivers are exempt, and the agency must review and approve each record.

Who must use EVV

Direct care workers who provide Medicaid-funded personal care or home health care services in New Jersey, through an agency or self-direction.

Since when

Personal care services since January 1, 2021. Home health care services since January 1, 2023.

How visits are recorded

A free mobile app on a smartphone or GPS-enabled tablet, or a call from the client’s landline. A device can be placed in the home where there is no cell signal or landline.

Where the data goes

HHAeXchange is New Jersey’s EVV aggregator. Agencies can use the free state-funded EVV tools or send visit data from their own EVV system.

Live-in caregivers

New Jersey does not require EVV for workers who live in the same home as the client, whether family or not.

Who checks the records

The agency remains responsible for reviewing visit records for accuracy and approving them for payment.

What does each EVV visit record?

Six details, captured electronically at every visit:

  • Person receiving the service
  • Person providing the service
  • Date of the service
  • Time the service starts and ends
  • Location of the service
  • Type of service

Location is captured only at the start and end of the visit, not during it.

Where it goes wrong

Why do EVV records break down?

Usually because of small, everyday slips that nobody catches until billing, when they are hardest to fix:

  • Caregivers forget to clock in or out, leaving visits incomplete.
  • Clock-ins happen late, so recorded times don’t match the schedule.
  • Last-minute shift swaps aren’t updated, so the wrong caregiver appears on a visit.
  • Nobody reviews exceptions until billing, when they are hardest to fix.

How HCSG helps

How does HCSG help with EVV?

HCSG’s virtual schedulers monitor clock-ins in real time, follow up right away on missed or late clock-ins, keep the schedule current after call-outs and swaps, and cover nights and weekends. Your agency keeps responsibility for approving visits and billing; HCSG makes sure the records are complete before they reach you.

Real-time EVV monitoring

Our schedulers watch clock-ins as visits happen, not at the end of the week.

Follow-up on missed or late clock-ins

When a caregiver hasn’t clocked in or out, we follow up right away and log attendance.

Schedule kept current

Call-outs, swaps and changed hours are updated promptly, so the schedule matches who actually made the visit.

24/7 coverage

Night and weekend visits get the same attention as weekday ones.

EVV FAQ

New Jersey EVV questions

Which home care visits need EVV in New Jersey?

Medicaid-funded personal care services (since January 1, 2021) and home health care services (since January 1, 2023), whether delivered through an agency or self-direction. New Jersey does not require EVV for caregivers who live in the same home as the client.

What does EVV record at each visit?

Six details: the person receiving the service, the person providing it, the date, the start and end times, the location and the type of service. Location is captured only at the start and end of the visit.

Which EVV system do New Jersey agencies have to use?

HHAeXchange is the state’s EVV aggregator. Agencies without an EVV system can use the free state-funded tools, which include a mobile app and phone clock-in; agencies with their own EVV system can send their visit data to HHAeXchange instead.

What if a caregiver doesn’t have a smartphone?

They can clock in and out by calling a designated number from the client’s landline. Where there is no cell signal or landline, a device can be placed in the home in limited circumstances.

Does EVV apply to private-pay clients?

The New Jersey EVV requirement covers Medicaid-funded personal care and home health care services. Many agencies still use the same clock-in process for every client to keep operations consistent.

How does HCSG help with EVV?

HCSG’s virtual schedulers monitor EVV in real time, follow up on missed or late clock-ins, log attendance and keep the schedule updated for call-outs and swaps, 24/7. Your agency keeps responsibility for approving visits and billing.
